A “medical home” is a process for care that unfortunately has a really confusing name. It has nothing to do with a physical home or medical facility. It has everything to do with building an environment of quality care and clear communication around your child’s medical needs. To create a medical home that meets the child’s medical and non-medical needs, the child’s family forms a partnership with the care team. This helps the family access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other services that are important for the health of the child and the family.
