The Medical Home

A “medical home” is a process for care that unfortunately has a really confusing name. It has nothing to do with a physical home or medical facility. It has everything to do with building an environment of quality care and clear communication around your child’s medical needs. To create a medical home that meets the child’s medical and non-medical needs, the child’s family forms a partnership with the care team. This helps the family access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other services that are important for the health of the child and the family. The Department of State Health Services, Children with Special Health Care Needs (CSHCN) and Texas Parent to Parent (TxP2P) have developed a Texas Medical Home Toolkit (PDF).
